For a finding of not guilty or dismissed in the interest of justice, issue the Blue Copy to the inmate and the White Copy to Records so records will know to remove the 804 and any other documentation pertaining to that RVR from the inmate's C-File. The Unit Office receives a copy of the final RVR for the disciplinary book The Chief Disciplinary Officer (CDO0 receives a copy of the final RVR. The CDO's office is the Associate Warden, which in turn will be sent to Master Registry who keeps track of all RVR's regardless of the outcome. The inmate has 30 days to file an appeal.
