accepted.

available at high school guidance offices.
Scores are received about three weeks
after test date.
CPT (Computerized Placement Test):
Students who do not submit ACT or SAT
test scores must take the Florida College
Entry-Level Placement Test (CPT).

dinator for accommodations before assess-
ment testing. Students with disabilities are
not required to disclose their disability to
the College; however, voluntary disclosure

HOW TO GET STARTED
1. APPLICATION FOR ADMISSION
Residency Information
Florida statute allows U.S. citizens and
lawful permanent residents to be classified
as a Florida resident for tuition purposes,
if the applicant or the dependent appli-
cant’s parent or legal guardian has been
a legal resident of the State of Florida for
at least 12 months preceding the first day
of classes of the term. To be eligible for
the in-state tuition rate, the claimant must
provide copies of at least two documents
with dates that evidence the 12-month
qualifying period. As some evidence is
TABE (Test of Adult Basic Education):
Students who are enrolling into a vocational
program must take the TABE test as required
for their program .
3. CPT and TABE testing will be given on
a first-come-first-served, walk-in basis.
CPT test sessions are on Tuesdays and
Thursdays at 9:30 a.m. or 1:30 p.m. TABE
test sessions are on Wednesdays at 9:30
a.m. or 1:30 p.m.
Review of skills before taking the test
is strongly encouraged and recom-
mended. Go to www.lakecitycc.edu/
test-center.aspx for test preparation
materials and practice tests.
Tests cover reading comprehension, sen-
tence skills, and mathematics (including
algebra on the CPT test). Review of these
skills can be beneficial to those who have
been out of school for a while. Helpful
resources include LCCC’s Collegewide
Learning Lab (CWLL), learning labs
at vo-tech schools, public school adult
education programs, libraries, and book-
stores.
The CWLL has test preparation materi-
als and can help to identify weak skill
areas and help stren gthen them. If
needed, the lab is an excellent resource
to build academic skills in preparation for
college courses. For hours of operation,

4. TEST ADMINISTRATION
Tests are given in the Test Center (Bldg.
015, Room 127). A government issued
photo ID and student identification
number are required. The CPT test is
untimed and takes approximately 1 1/2
- 2 hours to complete. The TABE test
is timed and takes approximately 1 1/2
hours to complete.
5. ADVISING & REGISTRATION
Based on your test results and intended
major, the test administrator will direct
you to the appropriate department for
advisement.

STUDENT ORIENTATION

Be one step ahead of the rest and start col- lege off on the right foot. Plan to attend the next Student Orientation. Call the Admis- sions Office (386) 754-4246 to reserve your space.

All students new to LCCC who have not participated in a Student Orientation are strongly encouraged to attend this highly informative session. Questions relating to scheduling, registration, and financial aid will be answered. Also, learn tips on surviving college and getting the most out of college life.

College-Level Academic Skills Test (CLAST)

Effective July 1, 2009, the legislation governing the CLAST requirements for students seeking an AA degree have changed.

In order to receive an exemption, students must meet CLAST exemption requirements using one or more of the following options: ACT scores, SAT scores, 2.5 GPA in specified coursework, or through a waiver based on special circumstances.

Once you are admitted to LCCC, or if you are a continuing student, advisors are available to help you:

CAREER SERVICES Students who have not declared a major should visit the Career Center in the Student Union Building (014) for help in deciding on their career. A Career Advisor is available to assist students in exploring their interests and matching those interests with careers. A computer assisted interest-based program called Florida CHOICES is available for career exploration. The Career Center can also provide help in finding employment and assistance in writing a resume. You can schedule to meet with an Advisor in the Career Center for career planning by calling (386) 754-4222 or (386) 754-4334.

REgISTRATIoN PRoCEDURES NOTE: Students who have holds on their records will not be allowed to register until these are resolved. These holds are generated as a result of obligations such as library fines, parking fees, or nonreturned equipment. All students must be fully admitted before they can register for classes. STEP 1 Pick up a copy of the Course Schedule or check LCCC’s Web site for schedule information and prepare a tentative class schedule. STEP 2 Make an appointment with your academic advisor. If you do not have an advisor, contact the Office of Advising and Student Develop- ment by calling (386) 754-4222. (This must be done before you can register.) Faculty advisors are not always on campus, so schedule your advising appointment early. STEP 3 Complete your initial registration form and obtain your advisor’s sig- nature on that form. Your registra- tion cannot be processed without an advisor’s signature. STEP 4 Take the signed registration form to the Registrar’s Office in Building

  1. Your classes will be entered

into the computer system and you will be given a copy of your schedule. STEP 5 Proceed to the Cashier’s Office to pay fees. If your fees are not paid by the due date, you will be dropped from your classes, and may not be able to reenroll due to classes being full. If you receive any type of financial aid please refer to the Financial Aid section in this schedule.

If fees are being paid by an off-campus agency, please read the Third Party Billing section of this schedule. If you have a scholarship from any individual or off-campus organization, please refer to the Foundation section of this schedule. Students without registration holds are eli- gible to register on the Internet, and are not required to complete steps 2-4. However, students are strongly encouraged to meet with an advisor regarding course selection. New studemts and dual enrollment students must meet with an advisor.

PAYmENT oF FEES

REGISTRATION FEES If the College has not received your fees by the designated deadlines, as shown on the Critical Dates Calendar on page 3, you will be dropped from your classes.

Fees for students who register for classes during the late registration period are due each day. (con't on pg 30) (from page 27) Students dropped for nonpayment must reregister, if space is available.

Students are strongly encouraged to pay early so that they do not have to face lines at the Cashier’s Office. Fees must be received by the designated deadline.

FULL-TImE/PART-TImE

STUDENTS

Full-time students are those registered for 12 or more credits each fall or spring semester or six or more credits during a summer term. Approval is needed for more than 19 credits in fall or spring or more than nine credits during a six week summer term.

THIRD PARTY BILLINg

STUDENT

You are a third party billing student if your fees are to be paid by a sponsor. A sponsor could be an employer, an off campus source, or the Florida Prepaid College Program.

If you are a third party billing student, you must take a copy of your registration form to Accounts Receivable in Building 015 to complete the registration process. Failure to do so may cause your registration to be cancelled.

If you add, drop, or withdraw from a class, you must give Accounts Receivable a copy of your add/drop form after it is processed by the Registrar’s Office. Third party billing students should verify with Accounts Receivable by June 23 by 3 p.m. that all paperwork has been completed. For additional information please call (386) 754-4208.

DEFERmENTS

No student’s registration will be guaranteed unless all tuition has been paid or properly

deferred with a financial aid award or veteran’s certificate.

Deferments are not automatic. All students, including financial aid students whose awards are not complete, who wish to apply for deferment, must do so at the Financial Aid Office.

Deferments may be processed up to three weeks before the beginning of each semes- ter. Specific criteria apply to be eligible for a deferment.

ADD/DRoP PRoCEDURE

Students have the ability to add or drop classes during open or late registration. However, an advisor must sign each add/drop form before it will be processed by the Registrar's Office. (1) Pick up an add/drop form from the Registrar’s Office (Bldg. 015) or Advising and Student Devel- opment (Bldg. 014).

(2) Complete the add/drop form and obtain your advisor’s signature.

(3) Return the form to the Registrar’s Office for entry.

All financial aid students must submit a copy of the Add/Drop form to Financial Aid.

Any student who has fees paid by an off-campus agency must provide a copy of their add/drop form to the Business Office (Bldg. 015). Students receiving funds coordinated through the Lake City Community Col- lege Foundation Office must give a copy of the add/drop form to the Foundation Office. Failure to do so may result in loss of aid. Students are encouraged to review their schedules to ensure that they are registered for the correct classes and sections.

FINANCIAL AID INFoRmATIoN

Application for Financial Aid If you are interested in applying for federal, state, or institutional funds, you should con- tact the Financial Aid Office in the Student

Union Building 014. It takes approximately three to four weeks to determine your eli- gibility for assistance after you have filed. Therefore, you are encouraged to start this process well in advance so that your financial aid eligibility can be determined and awarded to you prior to the time that your tuition is due. Otherwise, you should be prepared to pay your own tuition and fees. Steps for Officially Awarded Financial Aid Students If you have already received, signed and returned an official award letter to the Financial Aid Office, your tuition will be covered by your financial aid. If you are a Bright Futures Scholarship student , you will need to bring a copy of your registration to the Financial Aid Office so that you can complete a deferment form. Please see the Financial Aid Counselor, Wanda Baker, for assis- tance. This process could change due to Banner computer conversion.

Steps for Potential (Not Awarded) Financial Aid Students If you have applied for financial aid but have not received or signed your official award letter, upon completion of your registration process you should follow these steps:

  1. Go immediately to the Financial Aid Office to provide a copy of your registration. If you registered on-line, you should call (386) 754-4282.
  2. If you qualify for financial aid, your classes will be held by Financial Aid until an award can be processed for you.

Please be aware that if you have not signed an official award letter, you will be responsible for paying your own tuition and fees by the due date. Also, if your financial aid does not cover all of your tuition cost, you will be expected to pay the balance by the due date at 3 p.m. Failure to do so will result in your classes being dropped. This process could change due to Banner computer conversion.

authorize us either to use information from your check to make a one-time electronic fund transfer from your account or to process the payment as a check transaction. You also authorize us to collect a fee pursuant to Florida Law through an electronic fund transfer from your account if your payment is dishonored. To accomplish this, the College has elected to use an outsourced vendor to administer this function.

Checks issued to the College for tuition and fees which are subsequently dishonored by the bank and result in loss to the College will result in students being dropped from classes by the Registrar. The student will have 5 working days from the day of notification of the dishonored check to pay all College fees, and may then be reinstated with the approval of the student’s class instructors and appropriate Dean.

Dishonored checks which result in a loss to the College will immediately cause a hold to be placed on the student’s account preventing the student from registering or receiving transcripts until all outstanding fees are paid.

Adding classes: Students should remember that fees are due by the date and time indicated at the bottom of the processed registration form. If any course fees are unpaid at the time of the purge process, a student’s class(es) may be dropped.

Occupational Student Fees: Fees for occu- pational students in open-entry/open-exit programs are due at the time they register for classes. Students who have not paid at that time will be dropped from their classes.

NOTE: Fees are subject to change by the Florida Legislature and Lake City Community College District Board of Trustees without notice. Fees are affected by residency status.

FEES FoR RETAKINg

CoURSES

Florida State law mandates additional tuition/ fees for the third attempt at any college level

or college preparatory course. The increased fees will be the equivalent of the full cost of tuition and fees. To avoid the additional charges, you must drop a class prior to the end of add/drop. Withdrawing from a course after that time counts as an attempt.

Students should be aware that state law allows only three attempts in any college level or college prep class. A student who does not complete the same preparatory course within three attempts will be prohibited from registering for any college class , whether it is for college level or preparatory courses. For these purposes, a withdrawal or a change to an audit counts as an attempt. After three unsuccessful attempts in any preparatory class (or after an unsuccessful fourth attempt, if approved by the Vice President for Instruction and Student Services), a student will not be able to continue in any college class until appropriate remediation is sought from other sources. If you find yourself in this situation, speak with your advisor for appropriate alternatives.

A student may have only three attempts per college credit course, including the original grade, repeat grades, and withdrawals at any point. Students who do not complete the same college credit course within three attempts may not take that class for a fourth time.

Students who do not complete a course within three attempts may petition for a fourth attempt. The student must submit a written petition to the Vice President for Instruction and Student Services requesting permission to take the course for a fourth time. Students must pay full cost of instruction charges for all fourth attempts.

PURgE

The purge is a process in which the College does an accounting of all fees paid or deferred. Any student whose registration is not paid by the required dates will be dropped from his or her classes. Any student who registers during late registration must pay their fees by 3 p.m. on the day they register.

All students receiving some form of financial assistance, including financial aid, VA

benefits, Foundation scholarships, or third party sponsors, are required to submit their registration form to those offices after registration to avoid a possible purge of classes. If a student is dropped from classes due to nonpayment of fees, there is no guarantee that the same classes will be available if a student reregisters.

ATTENDANCE REVIEW All students are monitored for regular atten- dance at Lake City Community College. Students who register for classes, but who have never attended class by the end of the first week of classes, will be designated as never attending by the instructor and will be dropped from class rolls.

WITHDRAWAL PRoCEDURES If you stop attending class without with- drawing, you may be assigned an F for the class. Students who withdraw from a class are indicating that they no longer want to attend the class and are assigned a W grade for the course. Student initiated withdrawals:

  1. Obtain a Course Schedule Change form from the Registrar’s Office.
  2. Complete and sign the Course Schedule Change form.
  3. Obtain your advisor's signature.
  4. Inform your instructor that you are withdrawing from class. Also, if you are receiving financial aid, VA or a Foundation scholar- ship or third party payments, in- form the appropriate office.
  5. Return the form to the Registrar’s Office for data entry prior to the last date for student initiated withdrawals. After the student initiated withdrawal date, a student may withdraw only with the permission of the instructor, and must:
  6. Obtain a Course Schedule Change form from the Registrar’s Office.
  7. Complete and sign the Course Schedule Change form and ensure that your instructor and your advi- sor sign the form.

Also, Financial Aid, VA and Foun- dation Scholarship students must have the appropriate signatures of those offices before a withdrawal will be processed.

  1. Instructor must indicate approval by signature on the withdrawal form.
  2. Return the withdrawal form to the Registrar's Office for data entry when the appropriate signatures are collected.

NOTE: A withdrawal counts as an attempt in connection with the number of times a student takes a course. This may affect a student’s fees when the course is repeated. Also, there is no refund of tuition and fees when a student withdraws from classes. Students should be aware that a withdrawal from a course or courses may adversely affect their eligibility for financial aid or veteran’s benefits.

Upon the third or subsequent attempt at a course, a student may not withdraw and must receive a grade. Any withdrawal in a third or subsequent attempt will result in a grade of F****.

The last day for student initiated with- drawal from classes is designated in the critical dates calendar. Withdrawals may be made after this date by the instructor.

ADD/DRoP AND

WITHDRAWAL FoR

mINImESTER CLASSES

The add/drop period for minimester classes is up to the end of the day of the second class meeting. The add/drop period for weekend courses ends the end of the day on the Tuesday following the first class meeting. The latest date for a student initiated withdrawal is at the midpoint of the minimester sessions.

CoURSE CANCELLATIoN

Courses are contingent on meeting minimum class sizes.If a class does not meet minimum college enrollment requirements, the College reserves the right to cancel that class at any time.

REFUNDS

A student is entitled to a full 100% refund of tuition if a class is dropped before the end of the add/drop period. Refunds are calculated automatically by the Business Office and sent to the student approximately four weeks after classes begin. There are no refunds for stu- dents withdrawing after the official add/drop date. A refund for a minimester class will be processed if the student drops the class before the end of the day of the second class meeting or, in the case of a weekend class, a refund will be processed if the student drops the class by the end of day on the Tuesday following the first class meeting.

RECALCULATIoN oF gRADES

A student who receives a D, D+ or F in a course may choose to repeat the course to raise their grade point average. Lake City Community College will count only the last of up to three attempts of a class for grade point average purposes. Any subsequent attempts will be calculated in the grade point average. Students must submit a Request for Grade Recalculation form to the Registrar’s Office so that this may be done. This is not done automatically. Forms are available in the Registrar’s Office.

There are limits to the number of times a student may repeat a class and fees charged may increase for repeats above a certain number. Contact your advisor for more information.

BooKSToRE HoURS

T he College Bookstore hours for the fall semester are Monday – Friday 8 a.m. – 4 p.m. The bookstore will have extended hours at the beginning of the term. The College Bookstore is located in the Student Union Building 014. Students are advised to bring their student identifica- tion and a class schedule when purchas- ing textbooks to insure that the correct textbooks are purchased. Textbooks, as well as other supplies, may be purchased using MasterCard, Visa, personal check, cash or approved financial aid and third party book vouchers. Textbooks may be returned within the first seven days of fall classes for a refund. Students must show an original bookstore receipt with any return for a refund. New books must be returned unmarked and show no signs of use. The refund deadline for fall term is September 1, 2009. LIBRARY

The library hours for fall are Mon.–Thu., 7:30 a.m. - 7 :30 p.m., Fri., 7:30 a.m. - 4:30 p.m. Closed on Saturday, and Sunday. Bar-coded library cards are available at the front desk of the library.

Distance learning students have access to library databases and other online resources via the Internet at the World Wide Web address of: www.lakecitycc.edu, then select library.

CoLLEgEWIDE LEARNINg LAB

The Collegewide Learning Lab offers a vari- ety of services and programs to both day and evening students, as well as community mem- bers from LCCC’s five-county district. The lab provides computerized tutorial instruc- tion in writing, reading, math, and foreign languages. Peer tutoring is available for all developmental courses. WAIVER PRogRAm

Lake City Community College may waive tuition and some fees for state employees to enroll for up to six credits per term on a space available basis. State employees using the Tuition Waiver Program are required to sub- mit their Intent to Apply to the Registrar’s Office on or before three weeks prior to the registration date. See the critical dates schedule for dates.

The student must bring the registration form to the Registrar's Office before 3 p.m. on the last day of late registration and add/drop in order to register.

The Registrar is responsible for registering State Tuition waiver students. Students utilizing the State Employee Tuition Waiver Program will be registered for classes on the last day of late registration and add/drop. Students are responsible for any lab fees. Those fees are due the day of registration.

If the fees are not paid by the deadline, the student will be dropped for non-payment and may not use the State Fee Waiver for the class during that term.

This tuition waiver may not be used for any course being taken for a third or subsequent attempt or for a course offered as part of a limited access program.

ImPoRTANT DEFINITIoNS

FoR STUDENTS

SCHEDULE: This is a listing of the classes which you wish to take this semester. It should be developed with the assistance of your advisor. Advisors must sign all registration and add/drop forms.

REGISTER: This is the process whereby the Registrar’s Office inputs your classes into

the computer system to set up your schedule and determine class availability.

REGISTERED: Once classes are input into the computer system you are considered to be registered. However, you are not considered an official student until you have registered and paid for all classes.

DEFERRED : This term means that tuition is not due by the designated fee payment deadline. State regulations allow officially awarded financial aid students and veterans the ability to wait for actual receipt of funds before payment is demanded. Payment must be made within 60 days from the first day of the term. DROP: A drop means a student is eliminating a class from his or her schedule. When you drop a class you are entitled to a refund for that class. This can only be done during the open registration or add/drop period. Students can also be dropped by their instructor for never attending the class.

PREREQUISITE: This is a course that must be taken before registering for another course.

PURGE: If you do not pay your fees by the payment due date or make arrangements for financial aid or other assistance, you may be dropped from all your classes. This is called a purge. WITHDRAW: To withdraw means you are no longer going to attend a class. However, there is no refund of fees for a withdrawal. A withdrawal counts as an attempt in connection with the number of times a student takes a course. This may also affect a student’s fees for future attempts of that same class.

gRADUATIoN Students must submit a graduation application which has been reviewed, approved, and signed by their advisor. Graduation applications are available in the

Registrar’s Office. Students must complete a graduation application, in order to receive a diploma or certificate, even if they do not desire to participate in the graduation ceremony. In order for your graduation application to be processed in a timely manner, it must be turned in to the Registrar’s Office by the deadline on the Critical Dates Calendar on page 3. Graduation applications submitted after the deadline may be delayed in being processed.

Final examinations are held at the end of each semester/term. A grade of F may be assigned to any student who has an unexcused absence from a final examination.
