I. BACKGROUND INFORMATION
_____ is an ______ year old male/female in the ______ grade. The Gray Oral Reading test – 5th
Edition, GORT-5, was administered on _______ in order to assess oral reading rate and accuracy,
oral reading comprehension and total oral reading ability.
II. DESCRIPTION OF THE ASSESSMENT
The GORT-5 is a norm-referenced, reliable test that yields valid results for oral reading,
accuracy, fluency, and comprehension for individuals ages 6 years 0 months (6-0) through 23
years 11 months (23-11). The GORT-5 has two parallel forms (A and B), each of which contains
16 separate stories. Five comprehension questions follow each story.
Scores on the GORT-5 estimate the following oral reading abilities:
Rate. The Rate Score is derived from the amount of time in seconds taken by a student to read
a story aloud.
Accuracy. The Accuracy Score is derived from the number of words the student pronounces
correctly when reading the passage.
Fluency. The Fluency Score is a combination of the student’s Rate and Accuracy Scores.
Comprehension. The Comprehension Score is the number o questions about the stories that the
student answers correctly.
Oral Reading Index. TheOral Reading Index is a composite score formed by combining
student’s Fluency (ie. Rate and Accuracy) and Comprehension scaled scores.
III. OBSERVATIONS
_________ exhibited _______________ during the evaluation….
(Student behavior exhibited during the test.)
IV. SUMMARY OF THE ASSESSMENT
The Gray Oral Reading Test – Fifth Edition (GORT-5) was administered to assess __________’s
oral reading rate and accuracy, oral reading comprehension and total oral reading ability. The
GORT-5 also provides an analysis of oral reading miscues. Five types of miscues are analyzed:
meaning similarity, function similarity, graphic / phonemic similarity, multiple sources, and self-
correction. The scores for rate, accuracy, and comprehension are combined to form an overall
reading quotient. A summary of _______’s performance on the GORT-5 is presented below.

The following table outlines _____’s deviations from print while reading: III IMPRESSIONS AND RECOMMENDATIONS The results of the GORT-5 indicate that (name of student)__ scores were above/below/at the level expected for a student his/her age. All scores (accuracy, rate, fluency and comprehension) were all described as at/below/above average. _______’s overall reading quotient indicated poor performance. His/Her percentile rank of ______ indicated that ____% of the individuals her age performed at a higher/at level on this measure. These results, combined with information observations and deviations from print, indicate a need for enrollment in therapy. Therapy should focus on literacy, emphasizing comprehension, rate/ fluency and accuracy.
