Physical Assessment – should earn full points per rubric.

  • Auscultate lungs.
  • Inspect mouth/pharynx.
  • Look up nostrils.
  • Visual inspection AP chest
  • Palpate AP chest • Percuss AP chest.
  • Heart sounds
  • Visual inspection of extremities
  • Palpate extremities
  • Capillary refill
  • Blood pressure
  • Eyelid icepack test
  • Fundoscopic exam • Inspect/palpate head.
  • Ocular motor test
  • Muscle bulk/tone
  • ROM
  • Inspect neck.
  • Swallow
  • Gait and stance
  • Head rotation and shoulder elevation against resistance
  • Pharynx movement and phonation
  • Mandible muscles and mastication
  • Facial strength and expression
  • Look for involuntary movements.
  • Rapid alternating movements (hands & fingers)
  • DTR’s

  • Use of accessory muscles • Hx of HTN and DM controlled. DDX – will earn full points per rubric for differentials. MNM – definitely missing some but scored full points.
