Assessment Diary
Spring 2013 Assignments consisted of Drug Dosage Calculation & Clinical Evaluation Tool
1)The Drug Dosage Calculation exam was administered to entire N55 cohort
the first day of school January 2013. Students were notified prior to end of
previous semester which chapters to focus on In the Morris Calcuate with
Confidence text, along with assistance of skills lab faculty and materials for
practice with drug calculations. N55 cohort 13 students failed 1st try, 1 failed
2nd try, with 100% pass by 3rd try with score >90%.
clinical due to unsafe pattern of practice.
are expected to perform at 40% or better in each category, with >60% to
achieve full points (national average based). Client needs for provision of safe,
patient centered, evidence based practice are as follows:
o100% of students passed the Drug Dosage Calculation, however
multiple attempts. Plan to have 1st exam given at beginning of
55, then a second exam at end of 55 to see how students
progress with there ability to calculate and manage meds. Plan to
change problems to more reflect actual clinical orders for realism
& also to balance questions according to the MAPE policy created
this Spring to ensure content covered. Theory faculty to work with
Skills lab faculty to ensure students get in to skills lab to practice
drug calculations & provide improved contextual experiences
(med passing) for applying calculations to enhance conceptual
learning and relevance to practice.
oContinue use of Clinical Evaluation Tool pass/fail. Will tailor it
to Concept Model upon curriculum change.
oContinue to test with Kaplan. The Pharm test revealed the
following data need to emphasize safety and assessment in the
clinical environment. Since these are taught in 1st semester, will
discuss findings with 1st level faculty to work on these categories
in Fundamentals. The management of care is expected to be
rather low in this level of the program.

Spring 2012 a. We didn’t have a standardized plan of care across the program causing
students difficulty in adjusting to various formats, feeling inconsistencies.
b. 100% of students passed drug/dosage calculation test in 2 or less tries.
Two LVN-RN students had to retake and passed on the 2nd attempt.
c. Out of the seven clinical evaluation tools with a monkey survey; #8, #9,
#11, #19 and #21 are agree and strongly agree. #10 one out three
disagreed, #18 one out of three strongly disagreed and #22 one out of three
disagreed. Sample of 3 small, thus data somewhat skewed.
a. Developed standardized plan of care to be used across
curriculum. Reevaluate Fall 2013.
b. Doing Math review, and testing 2nd week of rotation to give
students more opportunity for success first time through.
c. Instructors need to increase application of theory in the clinical
environment, provide opportunities for critical thinking in the
clinical setting. Increase opportunities for students to increase
awareness of developmental differences, tasks, and crisis
situations through patient selection, and post conference activities.

b. Clinical instructors need to challenge students to think more
critically in the hospital setting, assist students in confidence
building by creating situations that provide successful outcomes in
hospitals and skills lab experiences.
c. Revise tests to meet course objectives more closely. Instructors
will evaluate classroom/seminar opportunities to challenge
students and improve critical thinking.

Spring 2013 98% students passed clinical rotation meeting objectives in clinical eval tool #5.
During clinical days, students reported to instructors and staff utilizing the SBAR
format. Additionally, students scored as follows on Kaplan testing:
Communication50.
1)Plan to create & post 3 forms of SBAR for next spring that
students with different learning styles can choose from to use.
Will ensure first post conference these tools are reviewed and
reinforced to decrease variability & help students capture most
important data components for reporting & communicating.
2)Some students were able to follow the resource or relief RNs
allowing them to fully see interprofessional communication. Plan
to get all students in N55 to have a day with this role to see the
importance of teamwork, simultaneously exposing them to the
QSEN competency of teamwork & collaboration, in addition to
patient-centered care.

Spring 2013 98% ofstudents passed #8 of Clinical Evaluation. 1/48 students failed to pass
professional standards at 2nd semester level, unpreparedness.
Students were exposed to electronic hospital information technology in their
hospital rotations. They met 8 hour course requirements per their facility
policy, and utilized electronic records for comprehensive patient data
gathering/planning/charting. Additionally the students completed focus
testing and 2 Integrated Kaplan tests on-line in preparation for NCLEX testing.
In discussing informatics with staff, faculty have decided to use the
Sim Chart product in the 2nd year of program as assignments to
learn e-navigation through charting. As faculty for 1st year, we
have opted not to use it yet, as students are exposed in their
facilities & are still learning the nursing process. Being that
SimChart is a relatively new program with bugs to work out, and
that existing electronics to be learned in 1st year are already
abundant; Healthstream for 4 modules, Certified background
checks for on-line uploading of their health & immunization
records, e-college, on-line Kaplan testing, hospital e-records,
electronic modules required by facilities, and e text book
resource tools, 1st year faculty feel Sim Chart would not add
benefit at this level. Will continue as students are meeting this
objective with all of the above in clinical and educational
exposure.

Legality and ethics 78.
Continue to evaluate students in clinical according to the
Evaluation Tool for this category 8, Legal, Ethical, Professional
Practice. May want to incorporate case studies regarding
malpractice cases, end of life patient care decisions into post
conference discussions for enhanced application.
Continue testing with Kaplan with questions pertinent categorical
