Nursing Program Application Procedures
1. All pre-requisites (except NSG 114) must be complete or in progress during the FALL or
SPRING semester prior to Nursing Program Entrance with a completion date of at the end of
the semester, unless written approval by Nursing Division Assistant Dean is on file.
2. Nursing Assistant Course (NSG114) must be complete by the end of the semester (including
the summer semester) prior to applicants entering the Nursing program.
3. Applicants are required to see academic advisor prior to applying to the program.
4. All transcripts must be on file and evaluated by the College Admissions Office prior to
appointment with the Nursing Advisor.
5. Advisors must verify that pre-requisites are complete or in progress for the appropriate
timelines set by the Nursing Department.
6. Advisors will complete a curriculum verification form for the applicant.
7. Applicants must pay the entrance exam $45 testing fee to the Business Office, then contact
the Testing Center (PRESCOTT = 776-2200, VERDE = 634-6561) to set up a testing time
before the Testing deadline. For FALL 09 admission this is 2/24/09.
8. The exam is computerized. You will have immediate test results which you will attach to
your application packet.
9. Study guide for the exam is available at the YC bookstore: HESI Admission Assessment
Study Guide, 2nd edition, ISBN 978-1-4160-5635-5.
10. All applicants must obtain a fingerprint clearance card. Please check the Nursing website
www.yc.edu/content/nursing for additional information or contact the Yavapai College
Campus Safety Office (776-2185).
11. Check this website for application dates. You need to include the following:
• Nursing Application for Admission
• Waiver of Licensure/Certification Guarantee Form
• Copy of Fingerprint clearance card
• Pre-admission test results
• Signed advising curriculum verification form
• A check for $35
