CTRS Assessments to know
Any assessment on a client involves gathering data from many sources. Sources for
assessment info can include but are not limited to: -
-The client
-The clients family
-The clients environment
-The clients history and background info such as age, diagnosis, education level
-contributions by other members of the treatment team (verbal and written)
-The clients medical record
-observation method
In assessment is completed two: -
To gather and analyze data in order to best determine the clients needs and
interest as well as strengths and abilities (remember that TR is ability oriented)
Culturally sensitive/competent -
Assessments need to be culturally sensitive. Therefore the TR needs to be
culturally competent
Comprehensive evaluation in recreational therapy - psych (CERT-Psych) -
a 25 item assessment that has one of the most widely used assessments in the
field of recreational therapy
It is an assessment made for short term acute care, psychiatric settings. It includes 3
areas of observation;
1. general performance
2. individual performance/experience
3 group performance
Helps determine behaviors that relate to a persons ability to successfully integrate into
society using social interaction skills
*Functional skills assessment
What does CERT psych help the therapist evaluate? -
The clients demonstrated functional level in the following areas: attendance,
appearance, attitudes towards rec therapy, coordination, posture, response to the
therapist (1:1), decision making ability, judgment ability, ability to form individual
relationships, expression of hostility, performance in organized activities, performance in
free activities, attention span, frustration tolerance level, strength/endurance, memory
for group activities, response to group structure, leadership ability in groups, group
conversation, display of sexual role in group, style of group interaction, handling of
conflict in group when indirectly involved, handling of conflict in group one directly
involved, competition group, attitude towards group decisions.

Three areas of observation (CERT-psych) - ✔1) General observation of the client

  1. The clients individual performance
  2. The clients performance within a group CERT-PD - ✔Intended for evaluating adult clients in rehab programs CERT-PD purpose - ✔To establish baseline levels of client functional skills needed for leisure activities. This tool is able to provide the TR'S with a good measurement of the clients functional abilities as they relate to leisure involvement. One of its strengths is that each sub Skal maybe used independent of the other. One of the drawback to this assessment is that administering the entire assessment takes a full hour or more to administer and frequently clients and physical rehab get too tired before finishing the full assessment assessment Leisure Diagnostic Battery (LDB) - ✔The LDB helps the therapist measure the clients perceived freedom as it relates to leisure and other factors that may be barriers to this freedom. It's available in paper and computer format with an accompanying manual that provides extensive information on the tools, Validity, and reliability The tools divided into two sections with a total of seven subscales -subscales: -perceived leisure confidence, -perceived leisure control, -leisure needs, -depth of involvement, -playfulness, -barriers to leisure experiences, -leisure preferences -Has both a long and short form has two versions, one for adolescence and one for adults
  • assess leisure Leisurescope (for adults) Teenscope (for adolescence) - ✔This is a quick test to administer clients to make 45 visual comparisons (pictures on slides or cards... Which do they like better?). Results are categorized into 10 leisure categories (games, sports, nature, collection, crafts, arts and music, entertainment, helping others/volunteerism, social affiliation, and adventure) along with emotional motivations for participation in activities Leisurescope/teenscope - ✔helps clients/therapist identify
  1. areas of leisure that are high interest to the client
  2. emotional motivation for participation in each of these areas of interest
  3. A patients need for high arousal experiences (desire and need to take risks) The state technical Institute's leisure assessment process (STILAP) -
  • functional skills assessment (access in the community, using social media, transportation) The general recreation screening tool (GRST) - ✔Is used for individuals with developmental/intellectual disabilities Measures physical, effective, and cognitive functions *Physical/cognitive assessment Minimum data set 3.0 (long-term care minimum data set) - ✔Medicare and Medicaid services required that the MDS assessment is conducted on all individuals in long-term care facilities TRS complete section F (customary routine and activities) When a CAT is triggered an individual receive a more in-depth assessment in this area
  • cognitive/leisure assessment MDS - ✔A standardized, inter-disciplinary tool designed for long-term healthcare facilities who is the residence receive Medicare or Medicaid benefits basically every client admitted to a nursing home in the US. The MDS is also sometimes referred to as the resident assessment instrument. It is used to conduct primary screenings and assessments of resident health status and includes a section for TR/RT responses. Idyll Arbor Activity Assessment (IIAA) - ✔There are four other instruments that fall under the umbrella of the 'Idyll Arbor activity assessment' these include leisure satisfaction measure(LSM), leisure attitude measures (LAM), the leisure motivation scale (LMS), and the leisure interest measure (LIM) The idyll arbor activity assessment (IIAA) - ✔Primarily intended for older adults living in long-term care LSM (leisure satisfaction measure) - ✔Helps the therapist determine the extent to which a client perceives his/her general needs are being met through leisure How the LSM is broken up - ✔To help the therapist pinpoint the specific areas that need Intervention the assessment divides leisure satisfaction into six subscales: -Psychological (sense of freedom, enjoyment, involvement, and intellectual challenge) -Educational (intellectual stimulation to learn about one's self and the world) -Social (rewarding relationships with other people) -Relaxation (relief from stress and strain of life) -Physical (means to develop physical fitness, stay healthy, control weight, and otherwise promote well-being) -Aesthetic (perceives that his/her leisure activities are pleasing, interesting, beautiful, and generally well designed) LAM (leisure attitude measure) - ✔Helps the therapist identify attitudinal barriers to the clients healthy involvement in leisure activities -Cognitive -Affective -behavioral

The cognitive component of LAM Gathers info on the following areas - ✔General knowledge and beliefs about leisure Beliefs about leisure's relation to other concepts such as health, happiness, and work Beliefs about the qualities, virtues, characteristics, and benefits of leisure to individuals such as: developing friendships, renewing energy, helping one to relax, meeting needs and self-improvement The effective component of LAM is designed to take into account - ✔Evaluation of his/her leisure experience and activities Liking of those experiences and activities Immediate and direct feelings towards leisure experiences and activities. This component generally reflects the respondents like or dislike of leisure activities The behavioral component of LAM - ✔Verbalized behavioral intentions toward leisure choices and activities and on self reports of current and past participation LMS (leisure motivation scale) - ✔Measures motivation relative to leisure involvement For subscales of LMS - ✔1. Intellectual - measures the extent of which the client is motivated to engage in leisure activities that involve mental activities such as learning, exploring, discovering, creating, or imagining

  1. Social- measures the extent to which the client engages in leisure activities because of the need for friendship and interpersonal relationships in the need to be valued by others
  2. Competency/mastery - measures the extent to which the client engages in leisure activities in order to achieve, master, challenge, and compete
  3. Stimulus/avoidance Dash measures the extent that a client needs to escape and get away from overstimulating life situations LIM (leisure interest measure) - ✔Therapist often complain about the common problem of using activity interest list in which the client just checks off activities that he/she likes results of such checkless are often questionable. LIM is a 29 item assessment that is more rigorous and measures how much interest the client has in each of the eight domains of leisure interest Eight domains or categories for LIM - ✔Physical Outdoor Mechanical

Tests ability to follow directions, orientation, cognitive decline over time, and ability to recall -scores may be affected by demographics such as age and education -Scores between 24 and 30 indicate normal cognition

  • cognitive assessment The therapeutic recreation activity assessment (TRAA) - ✔Assesses 2 to 4 clients at a time *Leisure assessment Seven assessment domains - ✔Sensory, cognitive, social, physical, affective, leisure, functional skills FACTR-R - ✔A 33 items screening tool used to determine the clients need related to his/her basic functional skills and behaviors. It helps to prioritize the clients treatment objectives. Once the therapist has reviewed the clients chart and has observed the client in a few activities, he/she would be able to fill out the FACTR-R score sheet. Doing so helps pinpoint the functional domain with the greatest need the ability to be improved. It helps the therapist identify the area most likely to improve through the services of recreational therapy The functional assessment of characteristics for therapeutic recreation - revised (FACTR-R) - ✔It recognizes that some functional abilities cannot be improved through TR services Measures pre-requisite skills related to leisure participation This assessment uses norm-referenced for scoring One of the few assessments that use patient's medical records in data collection

The FACTR/R Measures functional skills for leisure involvement in 11 areas within three domains - ✔-Physical, example:Ambulation general coordination, Vision, hearing etc. -cognitive example: attention, orientation, concentration, long-term memory, receptive language, etc. -social/emotional example: functioning in pairs, in small groups, and competitive situations, when involved in complex, or arguments etc. The inpatient rehabilitation facility patient assessment instrument (IRF-PAI) - ✔An assessment data collected on all Medicare part A fee-for-service patients who receive services under part A from an inpatient rehabilitation facility (IRF) at admission and upon discharge. IRF-PAI items addressed the physical, cognitive, functional, and psychosocial status of the IRF patients. The data collected for IRF-PAI is used for quality of care purposes and items developed primarily for IRS prospective payment systems Global assessment of functioning (GAF) - ✔This is considered one of the most often used scales for assessing impairment in clients with psychiatric disorders. Mental health and social work practitioners use it

during patient or caregiver interviews. It is a helpful tool for tracking individual patients clinical progress Life Satisfaction Scale (LSS) - ✔A 32 item scale. The LSS measures five dimensions of life satisfaction which provides the therapist with a singles score when they are combined LSS Five dimensions - ✔1)Pleasure verse apathy 2)Determination 3)Difference between desired and achieve the goals 4)Mood at time of assessment

  1. self-concept Glasgow Coma Scale (GCS) - ✔This is a measure used to describe the severity of a coma and subsequent injury by grading eye, motor and verbal responses. The numerical value of the scale range from 3 (low) to 15 (normal). The total of a clients GCS means: 3-8 severe coma, 9- moderate coma, and 13-15 mild coma. A newer Glasgow type scale has been developed for children which allows more accurate measurement of the early recovery in young children who are under the age of three.
