have prepared – hand written only, no copied sheets allowed. Using a copy of someone else’s reference sheet or
sharing of reference sheets is not allowed during the test. On each side of the reference sheet, draw a box in upper
right corner and write your name there. In the box, leave room for your instructor to sign his initials. There will be
a 15 point penalty if you fail to hand in your reference sheet with your test.
